Acne that has outlasted the pharmacy shelf and two GP courses needs a dermatologist and a plan that runs for months, not a single appointment. The programme is six months with Dr Nair, priced once, so that the reviews, the blood tests and the phone calls do not become reasons to stop.
What happens
- A forty-minute first consultation: acne type, scarring, what has failed, what you can live with
- Prescription treatment: topical, antibiotic, hormonal or isotretinoin, with the reasons written down
- Reviews at four, ten, sixteen and twenty-four weeks, in person or by video
- For isotretinoin: baseline and repeat bloods, a pregnancy prevention programme where it applies, mood and wellbeing questionnaires at every review, and Claire's number for anything in between
Rosacea follows the same shape with different medicines; the vascular laser for persistent redness is referred out and we say so.
Isotretinoin is the most effective drug for severe acne and the one with the most rules. We follow the British Association of Dermatologists guidance and the MHRA requirements in full; nobody starts it here without the bloods, the questionnaires and a proper conversation about the risks below.

00Monitoring and risks
Isotretinoin works, and the rules are what keep it safe.
Every risk here is screened for at every review: bloods, pregnancy prevention where it applies, mood and wellbeing questionnaires. The programme price includes all of it.
- 01Isotretinoin causes severe birth defects: contraception and pregnancy testing are compulsory where they apply
- 02Dry lips, skin, eyes and nose in almost everyone on isotretinoin
- 03Changes in mood, and rarely depression or suicidal thoughts: screened at every review
- 04Sexual side effects, including reduced libido and erectile difficulty, reported by some patients
- 05Raised cholesterol or liver enzymes, which is why the bloods are repeated
- 06Sun sensitivity and, for antibiotics, gut upset or thrush

01Why do you charge for a programme rather than per visit?
Because acne treatment fails when people stop coming. Per-visit pricing makes the fourth review feel optional; it is the one where the dose is adjusted and the scarring is assessed. £890 covers everything except the medicine itself, and it can be paid over twelve months at 0%.
02Can my teenager have isotretinoin?
Yes, from 16 with Dr Nair and from 12 with Dr Rahman, with a parent involved throughout. The MHRA rules on under-18s were updated in January 2026 and we follow the current version.
